The carrier is in business with an active operating authority for twelve (12) consecutive months with two (2) DOT inspections required
OR -
Twenty-Four (24) consecutive months with one (1) DOT inspection required

DOT only carriers require two DOT inspections regardless of length of authority

A safety rating of “Satisfactory” or “None”

Insurance of 100k Cargo and $1M Liability
